The Alzheimer’s Association may provide care consultations with an action plan as part of the Dementia Care Navigation Service. In addition, the Association will bring its expertise — including the 24/7 Helpline — and access to dementia care community education and support. The team at Rippl will provide specialized virtual dementia care.

What is
GUIDE?
The Guiding an Improved Dementia Experience (GUIDE) Model is a new Centers for Medicare and Medicaid (CMS) program that aims to improve quality of life for people living with dementia, reduce strain on their unpaid caregivers, and enable people living with dementia to remain in their homes and communities. This model provides individuals and families with an interdisciplinary team to provide care navigation, education, support and respite care.
The Dementia Care Navigation Service, powered by Rippl and the Alzheimer’s Association, delivers a patient-centric collaborative care model that meets all nine GUIDE care delivery requirements
Comprehensive assessment
Care planning
Ongoing monitoring and support
Medication reconciliation and management
24/7 helpline access
Care coordination and transitional care management
Referrals and coordination for services and support
Caregiver education and support
Respite services
Patients and caregivers receive on demand dementia specialty care at no cost including:
- Dementia diagnosis and education
- Tailored care plan, care navigation and community resources
- Clinical care, medication management
- 24/7 care team availability and crisis support
- Up to $2,500 in respite care for caregivers
